NANOTECHNOLOGY IN TREATMENT OF NEONATAL HYPERBILIRUBINEMIA

Apstrakt:
Hyperbilirubinemia (HB) occurs in 20-50% of term infants and more than 50% of preterm infants. Bilirubin in high concentration may exert its toxic effects on the central nervous system and cause the bilirubin encephalopathy which can cause death in 50% of newborns with HB. There are a few types of phototherapy (FT) used in treatment of HB and the most commonly used are: 1. The classic, in which a light source is a special blue fluorescent tubes, compact fluorescent tubes and halogen hotspots, and 2. LED (light-emitting diode), in which the LED light source is diode. Classic FT (light with wavelength of 457.9 nm - red, 476.5 nm - white, 488 nm - especially blue, 514.5-525 nm - green) changes molecules of unconjugated bilirubin in the blood vessels of the skin (by isomerization, oxidation, and addition) into reversible and irreversible (lumirubin) products which can be excreted by liver and kidneys. Recent studies favor LED over the classic FT. Nanotechnology has enabled grouping many LEDs in a small capsule called super LED. Blue super LED emits a high-intensity narrow band of blue light (450 nm) overlapping the peak spectrum of bilirubin breakdown. The unique blue super LED characteristics make it the most effective form of HB treatment.
